Ankle fracture: Treatment without surgery

Helse Stavanger offers conservative treatment for patients with stable ankle fractures where surgery is not necessary. The treatment involves early mobilization, gradual weight-bearing according to pain tolerance, and the use of a sling and crutches if needed. Patients are encouraged to move the ankle and toes early, with full weight-bearing expected within six weeks. After this, balance training and strengthening exercises are recommended, preferably in collaboration with a physiotherapist.

Follow-up is conducted digitally via CheckWare through questionnaires sent by SMS after approximately two weeks, twelve weeks, and one year. The responses are evaluated by healthcare personnel, and patients are only called in as needed. This ensures efficient use of resources and good patient safety. The service is suitable for patients with stable fractures and assumes the ability to participate in digital communication.

About the service

Helse Stavanger offers conservative treatment for patients with stable ankle fractures where surgery is not necessary. Studies show that such fractures can be treated without surgery with equally good results, as the ligaments help keep the ankle joint stable. The treatment involves early mobilization, elevation of the foot to reduce swelling and pain, and the use of painkillers such as paracetamol and ibuprofen.

Patients are encouraged to move the ankle joint and toes as soon as possible after the injury. Weight-bearing on the foot can be resumed within pain limits, usually with offloading using crutches during the first 10–14 days. Gradual increase in weight-bearing is recommended over the next 2–6 weeks, with the goal of using the foot normally after six weeks. After this period, patients should begin balance exercises and strengthening of the ankle joint, preferably in collaboration with a physiotherapist.

Follow-up is conducted digitally via CheckWare through questionnaires sent by SMS after approximately two weeks, twelve weeks, and one year. Patients' responses are assessed by healthcare personnel, and any need for further examinations is determined based on this feedback. Patients are also instructed to contact healthcare providers if they experience increasing pain, redness, or swelling that does not improve by elevating and offloading the foot.

This service is suitable for patients with stable ankle fractures that do not require surgical intervention and who have access to digital tools for follow-up. It combines efficient resource use with good patient involvement and provides flexibility for both patients and healthcare professionals.
