About the service
Helse Møre og Romsdal offers needs-based follow-up for patients with inflammatory bowel disease (IBD) at the outpatient clinic for gastroenterology in Ålesund. This service allows patients to register information about their own disease course through a digital online portal, which increases patient involvement in their own follow-up.
Traditionally, follow-up of IBD patients has consisted of regular check-ups at fixed intervals. With needs-based follow-up, patients can control when and how follow-up occurs, based on their own health status. This means closer and more timely follow-up without necessarily having to attend the outpatient clinic physically each time.
Patients receive an SMS with a link when it is time for self-registration and can log in to the online portal via smartphone, tablet, or PC using electronic ID (BankID). They answer digital questionnaires about their challenges every four months, as well as an annual quality of life questionnaire. The answers are reviewed by IBD nurses, who contact the patient if further evaluation is needed or arrange an appointment with a doctor or nurse. If no need for additional follow-up is identified, the patient continues with the next scheduled self-registration.
