Vestre Viken offers digital home follow-up via CheckWare for patients undergoing home dialysis, either peritoneal dialysis (PD) or home hemodialysis (HD). Patients complete a weekly self-registration form, and healthcare personnel review the responses to adjust treatment as needed. Two-way message exchange between patient and provider is also available, and for PD patients using a night machine, treatment data is transferred directly to the hospital.

The service provides patients with flexible and close follow-up while allowing healthcare personnel to continuously monitor the treatment and intervene when necessary. This contributes to increased patient safety, earlier identification of complications, and better resource utilization in healthcare services.

Vestre Viken Health Trust offers a digital home follow-up service for patients undergoing home dialysis, either peritoneal dialysis (PD) or home hemodialysis (HD). This service is developed to supplement regular hospital visits and enables closer monitoring of the patient's health condition and dialysis treatment performed at home.

Patients complete a weekly digital self-registration form that provides information about their condition and the execution of dialysis treatment. These responses are reviewed by healthcare personnel at the hospital to assess the need for any treatment adjustments. The system also facilitates two-way message exchange between patient and provider, with messages from the provider notified on the patient's phone.

For patients using a PD night machine, treatment data is transferred directly from the machine to the hospital. This gives healthcare personnel an overview of completed treatments and the ability to adjust the treatment program without the patient needing to physically visit the hospital. This way, any complications or issues can be identified early, and necessary measures can be implemented quickly.
