Follow-up of patients undergoing hip prosthesis surgery has traditionally taken place through physical appointments. By moving this check-up from a physical appointment to digital follow-up in CheckWare, the patient can avoid physical attendance and report on their health status at home using digital assessments. Using digital tools based on patient responses, the healthcare provider can contact the patient if needed.
About the service
Digital follow-up in connection with hip prosthesis surgery helps to free up more time to treat those who need physical appointments at the hospital. The purpose of using a digital platform for follow-up is to provide the patient with the same quality of follow-up at home without compromising the healthcare services offered.
Using automated assessment plans, the appropriate assessments are assigned at the right time, and the patient receives notifications via SMS when new assessments need to be completed. The patient completes a total of four questionnaires concerning their health status during the treatment process. Questionnaires are filled out approximately one week before surgery and again six months after the surgery has been performed. A combined report (summary report) compiling the responses will be automatically generated and, if integrated with the patient record, transferred to the journal. This allows the healthcare provider easy access to the report and the ability to compare results and the need for further follow-up.
The summary report uses color codes to alert responses outside given threshold values, which may indicate a need for further follow-up by healthcare personnel. In the healthcare personnel’s user interface, flags are also raised if the patient has submitted responses that require attention. In the patient’s user interface, some responses will also provide direct feedback on the need for measures, with colors and textual content.
