Heart Failure - Digital Home Monitoring

Helse Nord-Trøndelag HF offers digital home follow-up for patients with heart failure, developed to provide more flexible and continuous monitoring of their health status. Through a secure web portal provided by CheckWare, patients can record blood pressure, pulse, weight, quality of life, symptoms, and medication use on an agreed day between 6:00 AM and 12:00 PM. Nurses review the weekly entries and provide feedback via SMS, and if necessary, it may be required to adjust medication plans or escalate treatment.

The service is especially suitable for patients who want a more active role in their own treatment and who need frequent follow-up without having to attend the hospital in person. If needed, patients can contact the heart failure outpatient clinic directly.

About the service

Helse Nord-Trøndelag HF offers a digital home follow-up service for patients with heart failure, developed to provide more flexible and continuous monitoring of their health status. Through a secure web portal provided by CheckWare, patients can record blood pressure, pulse, weight, quality of life, symptoms, and medication use on an agreed day between 6:00 AM and 12:00 PM. To participate in the service, patients must have access to a smartphone, PC, or tablet, as well as a blood pressure monitor and scale. Patients using multidose medication or requiring an interpreter are not offered this follow-up. The service is especially suitable for patients who want a more active role in their own treatment and who need frequent follow-up without having to attend the hospital in person. If needed, patients can contact the heart failure outpatient clinic directly.

By using patient-reported outcome measures (PROM), the information is collected into an anonymized registry that contributes to developing knowledge about what works in treatment. This enhances the quality of services and provides better decision support for both the patient and healthcare provider.

Digital home follow-up is an alternative to traditional outpatient controls and gives patients the opportunity to decide when and how follow-up should take place. It is particularly suitable for patients who want a more flexible and proactive approach to their treatment. Patients can at any time discontinue the digital follow-up and return to regular outpatient follow-up if they wish.
