Helse Møre og Romsdal now offers a digital, needs-based follow-up for patients with type 1 diabetes as an alternative to traditional, fixed outpatient clinic appointments. This service allows patients to record information about their disease progression via a secure web portal, which can lead to closer and more tailored follow-up without necessarily having to meet physically at the hospital each time.
Patients receive an SMS with a link to the login page when it is time for self-registration, which occurs every four months and at the annual check-up. Login is done via ID-Porten with electronic ID (BankID), and the registration is completed on a secure web portal provided by the subcontractor CheckWare. After logging in, the patient completes a digital questionnaire with 15-60 questions about quality of life, challenges, and treatment. A diabetes nurse reviews the answers, and if necessary, contacts the patient by phone for further assessment or schedules an appointment. If the answers do not indicate a need for further follow-up, the patient receives a new questionnaire four months later. The patient can also voluntarily complete the questionnaire if there is a change in their situation.
This service is suitable for patients who want greater flexibility and autonomy in their follow-up and who have a stable type 1 diabetes condition. Patients are free to discontinue the digital follow-up and return to regular outpatient follow-up at any time.
