The Osteoporosis treatment consists of automated assessment plans that simplify administrative work, a specially developed provider form for recording measurements, and an associated report with evaluation, treatment needs, and follow-up of osteoporosis. The comprehensive data foundation provides decision support for treatment of fractures as well as reporting to the low-energy fracture registry for quality assurance of the treatment.
About the service
Osteoporosis uses planning tools in CheckWare for automated sending of forms, SMS notifications, and clearing of unanswered forms at selected intervals. The assessment plans include, among other things, forms to be completed by the provider as well as follow-up forms filled out by the patient during the course of treatment.
The provider form is unique to the treatment and consists of two parts: the first part contains patient-reported information. The second part includes recording measurements related to the consultation, such as fracture anamnesis, blood test measurements, and bone mineral density values. The provider form has a responsive design, show/hide logic based on responses, and red indicators for abnormal values. The accompanying report summarizes notes from the consultation and any assessment regarding patient follow-up. Response values in the completed provider form are used as a basis for further follow-up and possible pharmacological treatment.
Home follow-up of the patient facilitates needs-based follow-up with the option of consultation with a nurse if needed. This way, time is freed up for both patient and healthcare personnel when no further follow-up is required.
Data from the treatment can be reported to the BeinOP low-energy fracture registry, which helps ensure quality assurance of fracture and osteoporosis treatment.
