SSF-IV – CAMS Suicide Status Form

Suicide Status Form IV (SSF-IV) is now available in EG CheckWare's assessment library. The form is a unique, multi-use clinical tool that guides the therapeutic framework for suicide-specific evaluation and treatment of a patient's suicide risk.

After agreement with the rights holder, we inform that the assessments must be assigned individually and not through a plan, due to the dynamic nature of the treatment.

Detailed information:

SSF-IV forms are assigned manually due to the dynamic nature of the treatment. These are "collaborative assessments" that build trust and engagement between patient and clinician. The SSF-IV solution in EG CheckWare consists of 4 assessments:

1. SSF-IV - CAMS Suicide Status Form (initial session)
Part A – The patient completes Part A which includes assessment of pain and reasons for living and dying
Section B – The clinician asks questions to the patient and completes the treatment plan
Section C – The treatment plan
Section D – CAMS stabilization plan includes limiting access to lethal means, ways to behave differently in a suicidal crisis, emergency contacts, and removal of barriers to treatment participation.
Two reports are generated for the assessment.

An additional report containing a summary of the treatment plan and stabilization plan can be printed and signed by both clinician and patient.

2. SSF-IV - CAMS Suicide Status Form (interim session)
Section A - The patient assesses and works with drivers. The clinician uses their knowledge and skills to apply techniques to treat each driver.
Section B - The session ends with reviewing and updating the CAMS treatment plan.
Can be assigned multiple times, as needed.

Two reports are generated for the assessment.

An additional report containing a summary of the treatment plan update can be printed and signed by both clinician and patient.

3. SSF-IV - CAMS Therapeutic Worksheet
Incorporation of the CAMS Therapeutic Worksheet (CTW) is optional and should be properly used and referenced in subsequent sessions as needed.

SSF-IV-CTW can be used in later interim sessions or revisited as needed during CAMS interim care.

4. SSF-IV - CAMS Suicide Status Form (outcome/disposition final session)
During the final session of CAMS, the clinician and patient agree that the patient is able to manage their suicidal thoughts and feelings.

Next steps may include determining an optimal outcome and disposition.

Two reports are generated for the assessment.

An additional report containing a summary of the outcome/disposition can be printed and signed by both clinician and patient.
