Healthcare professionals at the hospital conduct a needs assessment for the entire family before discharge, which enables a tailored treatment plan. With the treatment plan and assigned technical equipment, parents can report observations, measurements of vital parameters, and nutritional intake for the child. For example, pulse, temperature, weight changes, breastfeeding frequency, and general behavior can be measured. Integration with the medical records system ensures that reported observations and measurements are automatically recorded in the child's chart.
At the start of the home stay, the reporting frequency will be about twice a week, and healthcare professionals continuously monitor the child's development. Through secure messaging in CheckWare, healthcare professionals and parents can communicate daily and as needed, and parents can use the diary function to report on the situation at home.
